I give my consent to Davianna Sierra, of Pretty Ugly Tattoos LLC, to perform a tattoo, permanent makeup procedure or body piercing on me. In consideration of doing so, I release Davianna Sierra and Pretty Ugly Tattoos LLC from all manner of liabilities, claims, actions and demands, in law or in equity, which I or my heirs might now or hereafter have by reason of complying with my request.
I understand that no employee of Pretty Ugly Tattoos LLC acts in the capacity of a medical professional when performing a tattoo, permanent makeup procedure or body piercing. Any suggestion they make is a suggestion only. It is not to be taken as, or substituted for, advice from a medical professional.
I understand the procedure will be performed using appropriate techniques, instruments and pigments. I also understand that infection can happen from a lack of proper hygiene, and that a reaction to a pigment is possible. So that my tattoo, permanent makeup or piercing heals properly, I agree to follow the written and verbal aftercare instructions I am given until healing is complete. I understand healing can take several weeks.
I understand that I am making a permanent change to my body, and that no claim about the possibility of reversing that change has been made or implied by Pretty Ugly Tattoos LLC or by anyone working with it.
I have read this release and I confirm that everything I have put on this form is correct and complete. I understand that this is a release and I agree to be legally bound by it.
